This application is available from Wednesday, August 17, 2005 at 10:00 AM to Tuesday, August 30, 2005 at 5:00 PM.

To file the application, you must use your unity account to log on. For information on your username and password, please visit the ITD Unity Account Help Pages.

This form can also be downloaded and submitted in print to the Student Government Office, 307 Witherspoon Student Center.

Candidates declaring online must complete a printed Intent-to-Run Form Side B (agreement/wavier) at the All-Candidates Meeting, Wednesday, August 31st.

Instructions and Regulations:

All instructions and regulations concerning this form must be followed, or it shall be considered invalid and your candidacy voided.

  • All spaces must be filled out, unless otherwise noted.
  • The “Full Legal Name”space must contain your name as officially appearing on University records.
  • When filing for Senate seats, please include your preferred seat. However, candidates may only run for seats they are eligible for, and the board will move all candidates to the appropriate seat.
  • In the “Ballot Name Format” section, include your name exactly as you wish it to appear on the ballot. This name format must include your surname from your full legal name; it may also only contain other portions of your legal name or common derivatives thereof, and nothing else.
  • All candidates must submit both side A (in print or online, if applicable) and side B. Both sides must be submitted for the filing to be complete. This form is due Tuesday, August 30th 2005 at 5:00 PM; however, candidates filing online may submit side B as late as one week following the all candidates meeting.
  • Candidates are responsible for ensuring the board has properly received and processed the form. All receive and processed forms are made public by the board. Individuals failing to alert the board to errors in these regards within 24 hours following the all candidates meeting shall forfeit any claim to candidacy.
